UPDATE ON NEW MEDICARE THERAPEUTIC SHOE PROGRAM
RULES
- The CMN 4.01, Certificate of medical
necessity has been replaced by the simpler:
Statement of Certifying Physician. (enclosed)
- Podiatrists can order shoes and
modifications, but only the primary physician
and or endocrinologisrt cab sign the
certifying statemeny letter.
- A custom molded shoe is covered **ONLY** when
the patient has a foot deformity which cannot
be accommodated by a depth shoe. THE NATURE
AND SEVERITY OF THE DEFORMITY MUST BE WELL
DOCUMENTED BY THE PHYSICIAN AND PODIATRIST,
AND MUST BE KEPT IN THE SUPPLIER'S RECORDS.
- It is unclear whether other problems and
special work, such as build-ups for short
limbs, will be covered.
We appreciate your referrals and will do our best
to keep your paper work requirments to a minimum.
